Make a correction on the dates but forget to ask the borrower to
initialize the change.- Borrowers signs "I wish to cancel" section
- After signing on wrong line, borrower signs on the right line but fails to cross out the first signature and inital the correction

- Borrower did not sign at the top of the first page.
- Page 4 with no signatures

- Borrower changed APR (very unusual)
- Date missing next to signature at the bottom.
